What Is Social Prescribing?

Social Prescribing means we connect people to non-medical community supports that improve overall well-being. These supports can vary widely from person to person, and can include community activities, programs, and supports.

Whether you're facing isolation, health challenges, or simply need help navigating local resources, our team takes time to understand your unique needs and match you with the right programs, activities, and services.

When people have things to do, places to go, and people around them, it can reduce isolation and help prevent crisis. Being part of a community can help us feel seen, supported, and connected. Examples include:

  • Joining a card group or social club

  • Taking a dance or fitness class

​From walking groups and mental health resources to food supports and social connections, we're here to help you feel more connected, supported, and empowered in your community. Whether you're

facing health concerns or simply want to feel more connected

and supported, we're here to help you find what fits your life.
