About Us

Our services focus on mental health & wellness. We also support older adults &

their care partners.

We are a not-for-profit organization that brings together local community partners to collaborate on service provision and address the social determinants of health. We work to educate, coordinate and facilitate access to existing programs and services and create new ones to fill identified gaps.

We are a small, dedicated team working alongside local organizations to provide resources, programs, and support to residents of the Eastern Shore and Musquodoboit Valley. The initiative began in 2020 as a pilot project under Harbourview Lodge and the The Old School. In 2024, we formalized our status as a nonprofit, strengthening partnerships to better serve our communities.

We help community members age in place and navigate transitions in care. Through our work, we aim to foster a connected, supportive community where everyone can experience hope, dignity, and belonging. Our services are supported by our Board Members, representing communities throughout the Eastern Shore and Musquodoboit Valley, who contribute their diverse range of professional expertise to our organization.
