One-on-One support for seniors and caregivers

Michelle works with individuals and families in many ways, including supporting clients in navigating the health care system and accessing the resources they need. She also connects individuals with broader community supports through social prescribing.

Aging in Place

Michelle supports seniors and their loved ones in planning for aging in place. Aging in place means helping people remain safely and comfortably in their own homes as they grow older. This can include coordinating home supports, identifying safety upgrades, arranging in-home care, and securing funding that allows individuals to maintain independence while staying connected to their community.

 

In addition, Michelle provides guidance and emotional support to families when a loved one receives a new diagnosis, helping them understand their options and access appropriate services.

Michelle can assist with:

  • Navigating health, aging, and government systems

  • Completing applications (Service Canada and other government programs)

  • Accessing food and other basic supports

  • Arranging transportation for medical appointments

  • Finding private caregivers and foot care providers

  • Securing funding and benefits such as:

    • home repair grants

    • senior care grants

    • heating rebates

    • property tax rebates

  • Connecting to local programs, social activities, and community services

  • Identifying benefits and resources that support aging in place and unpaid caregivers
